How it works
- For routine eye exams, you can see an eye doctor without a referral from your primary care provider.
- Pick an in-network eye doctor close to home.
Children, teens, and young adults (birth through age 20)
- One eye exam and prescription eyeglasses once every 12 months.
- You may qualify for additional services if your vision changes.
- More services may also be approved if requested in writing by the child鈥檚 primary care provider, teacher, or school nurse.
Adults (age 21+)
- One eye exam once every 24 months.

As a value-added service, PCHP offers a $100 allowance every 2 years to upgrade your frames鈥攕o you can see clearly and feel great in your glasses. (Not available for CHIP Perinate members.)
