What is interoperability?

An easy, secure way to exchange, access, and integrate health information across systems. Access may be within any health system. The app accesses the insurer鈥檚 system to get health鈥痠nformation.

Interoperability ensures that your health care insurer is compliant with federal government guidelines. Members get easy, secure access to their health鈥痠nformation.

How can I access my health information

Follow the steps below to access your health鈥痠nformation. Already connected to鈥疨CHP:

  • Choose your preferred application(s) like鈥疧neRecord
  • Log in to this approved application. Provide consent to access your health鈥痠nformation.
  • Per CMS guidelines, the consent provided in your preferred applications(s) will be valid only for 90 days. You will have to provide/renew consent every 90鈥痙ays.
  • Have not yet connected鈥痶o PCHP:

    • Choose your preferred application(s) from the list of鈥痑pproved applications鈥痑nd download from the Apple App Store or Google鈥疨lay Store.
    • Log in to the approved application. Provide consent to access your health鈥痠nformation.
    • If the application you select is not already connected to PCHP, it will need to go through an approval and registration process with PCHP. Once that process is complete, you will be notified to log in to the approved app and provide consent to access your health鈥痠nformation.
    • Per CMS guidelines, the consent provided in your preferred applications(s) will be valid only for 90 days. You will have to provide/renew consent every 90鈥痙ays.
    • Note:鈥疘t may take up to 24 hours to see your health information after you鈥檝e registered and opted in to third-party applications.

What health information can members access?

Is the data current?

Per the CMS guidelines, members can access the latest health data 24 hours a鈥痙ay.

How much health history can members access?

Per CMS guidelines, members can access data that is up to 5 years old.

How do I stop the application from accessing my health information?

Log in to the app and revoke consent. Once consent is revoked, your health information is no longer accessible on the鈥痑pp.

Can I access my child鈥檚 health information?

Yes. Parent(s) or caregivers can access their child鈥檚 health information if they are listed as a Legal Authorized鈥疪epresentative.

Using a Third-Party Application to Access Your Protected Health Information

If you want to use a third-party application (鈥淭hird-Party App鈥) that connects to 最新糖心Vlog Community Health Plan to access your protected health information (鈥淧HI鈥), please read this important information.

最新糖心Vlog Community Health Plan requires鈥疶hird-Party Apps鈥痶o provide us written confirmation that they meet certain requirements when accessing 最新糖心Vlog Community Health Plan member PHI. This is called an attestation. It also gives us confirmation that the鈥 Third-Party App鈥痯rovider agrees to specific requirements to keep PHI confidential and鈥痵ecure.
