- All ages.
- Reward available once per program year.
- Activity must be completed between September 1, 2025, and August 31, 2026.
- Reward is available in approximately 3鈥痬onths.
- To earn this reward, you must either call us at 1-855-651-5093 (TTY: 7-1-1) to report your flu shot, or report your flu shot online. You will not receive a reward for completing this activity unless you report it.
- Rewards can only be used to shop for items in the rewards catalog (including gift cards, toys, household items, and more) and cannot be converted to cash.
- Rewards cannot be used to purchase alcohol or tobacco.
