×îÐÂÌÇÐÄVlog Community Health Plan (PCHP) was established as a managed care organization in 1999, when our sponsoring organization, ×îÐÂÌÇÐÄVlog Health, saw a need to extend its mission and offer high-quality, low-cost care to Dallas County residents without private health insurance. With the support of local community leaders, PCHP began providing services to recipients of STAR (the Texas Medicaid managed care program) and the Children’s Health Insurance Program (CHIP). By offering these programs, PCHP helps members access services that are designed to meet their specific needs and improve their overall health.
Through the PCHP network, members can access convenient, affordable healthcare and trust that they have a place to turn for safe, high-quality services for themselves and their families. Today, our service area covers Dallas, Collin, Ellis, Hunt, Kaufman, Navarro, and Rockwall counties, where members can seek care from more than 9,000 in-network primary care doctors and specialists.
Our plans provide access to regular preventive care and assistance in managing chronic conditions. Through an innovative, person-centered approach, we aim to reduce the need for emergency room visits and improve the overall health of the communities we serve.
We also understand the importance of addressing the social needs that negatively impact health and well-being, including barriers to transportation, housing, and food, among others. To do this, PCHP offers holistic service coordination with attention to cultural awareness, spiritual beliefs, health literacy, and person-centered service planning. We also leverage our strategic community partnerships to develop innovative solutions to important health issues facing our seven-county service area.
