Asthma Be in Control Reward Details
- All ages.
- Must have a diagnosis of asthma
- Must be enrolled in the Asthma Be In Control program for鈥�6鈥痬onths in a row.
Medication Reward Details
- Ages 5 and up.
- Must have a diagnosis of persistent asthma.
- For prescribed long-term controller medication only.
- Does not include rescue medications.
- $15 value per 90-day refill.
- $60 value maximum per program year.
General Reward Details
- Rewards are available in approximately 3鈥痬onths.
- Activity must be completed between September 1, 2025, and August 31, 2026 to earn this reward.
- Rewards can only be used to shop for items in the rewards catalog (including gift cards, toys, household items, and more) and cannot be converted to cash.
- Rewards cannot be used to purchase alcohol or tobacco.
